Download, save, and fill out the Medication and Immunization Forms linked below. Upload all completed forms to your Parent Portal.

​Certificate of Immunizations – Colorado State Child Care Licensing requires that your child’s immunizations record (including for COVID) be on file in our office. The official Certificate of Immunization must be from one of the following states: Colorado, Alabama, Arizona, California, District of Columbia, Florida, Georgia, Illinois, Iowa, Kansas, Kentucky, Maryland, Montana, North Dakota, Oregon, Pennsylvania, South Dakota, Tennessee, Texas, Virginia, or Washington.

*If your child’s vaccination record is from a different state or country, please print the Colorado Certificate of Immunization at this link. Copy your child’s immunization record by hand onto the Colorado form and submit it to our office. This is a Colorado State Childcare Licensing requirement.
